Clinical Integration

Clinical integration is the alignment of the HPHO medical community around common clinical goals and strategies designed to promote high quality, cost effective healthcare, and the ability to measure and compare outcomes for the population of patients we manage. Current clinical integration activities focus on expansion of the disease registry program using the MedVentive registry engine, outpatient care management and pay-for-performance initiatives.
